Please note that while we make every effort to provide you with the best care possible, there are limitations and restrictions that your health insurance plan may impose. As a result, we ask that the following policies and guidelines be reviewed prior to your visit to help ensure an easy and efficient experience.

- New patients are asked to arrive 20 minutes prior to their appointment, to ensure that all office paperwork can be processed in a timely manner.
- New Patients are required to bring their insurance card and a photo identification. Similarly, if a patient has changed their insurance an insurance card and photo ID are required.
- Referral/Authorizations: While our office will assist or in some cases initiate a referral or authorization on a patient’s behalf, it is ultimately the patient’s responsibility to ensure that referrals/authorizations are received ahead of their scheduled appointment.
- Cancellation policy
- We require a 24-hour notice to cancel or reschedule an office visit appointment.
- All diagnostic testing such as VNG, Allergy, Audiology and CT requires a 72-hour notice for cancellation. Failure to notify the office in a timely manner will result in a cancellation fee of $100.00.
- All surgeries in the out-patient setting as well as in-office procedures requires a 72-hour notice for cancellation. Failure to notify the office in a timely manner will result in a cancellation fee of $100.00.

- Office hours are 8:00 a.m. - 4:30 p.m. Our phones are on service (for emergencies only) from 4:30 p.m. until 8:30 a.m. (the following morning).
